Optionally configure internal service via its load balancer

This PR adds the option to configure a service that's not internet facing.
It will use an internal load balancer instead of internet facing, living in the private subnets instead of public ones

Is this still maintained? It's a great module but there doesn't seem much activity lately wrt outstanding PRs. We can keep developing on a fork but it would be nice to have one source of truth. Thanks!
