Ollama Extra Configuration

The Ollama integration works fine for me, even with the ollama cloud models. However, I have ollama hosted on a home lab and on different providers as well.

I would love to have the ability to:

• Change the host of the ollama provider in the settings
• Add multiple ollama provider connections

Ha! I disabled multiple providers for localhost based providers assuming folks would only have one. This is an easy change and I'll do it.
