VS Code and Insiders apps should use coder-remote extension URIs

Currently they just use the SSH extension directly rather than the coder-remote extension.

The coder-remote extension requires owner=xyz, and if you supply owner=me it will create a new history record.

The coder-remote extension requires owner=xyz, and if you supply owner=me it will create a new history record.

WIthout this change, it's still two history records if you also use the extension, but one of which is just the Coder Connect hostname.
