Did you try the annotation tools at the bottom of the tool pane? The arrow icon and the text icon.

If so, what changes would you recommend?

In you did not see them, do you perhaps have an idea how to increase their discoverability?

What would this "Comment" look like?

You can type comments onto the workflow (using the T button).

Also, widgets have a report button at the bottom. You can add comments into the report, e.g. "In this graph we can see that ..."

As said, Orange already allows adding comments.

This example from a commercial product apparently provides formatted text and images. Orange doesn't and won't contain a rich text editor.
