Project load progress appears like nothing is happening

Currently, when a project is loading (in terms of extensions), or re-loading after a change, there is no visible indicator that anything is happening, which can cause user confusion.

We need to show a progress indicator of some form (ideally using the standard VS Code mechanisms) to indicate that we are working on stuff.
