[BUG] Table filter conflicts #42

Describe the bug
Two or three types of conflicts noted. [1] appears to include both a backend issue (/x=True&x=False/ in URL) and frontend issue. [2a]-[2b] may be just frontend. Steps to reproduce in parentheses, with screenshots included:

[1] Choosing affirmative then negative for same filter appears to apply both. Only the more recent choice is actually applied, but checkmark and X are chained in URL and both marked active.

[2a] Some options mark an incorrect choice in filter bar, though URL and results are correct.

Shows 69 of 232 records (all Telehealth Yes, but filter bar says active filter is "MAT prescribed")

[2b] "Start Over" does not consistently reset filter-bar markers to inactive state.

Shows 232 of 232 records again, but "MAT prescribed" filter is still marked as active
