[BUG] OTP table certification date blank

OTP table’s Certification Date col all blank [siterecs_samhsa_otp, full_certification])

@donguyentung2001 @SamuelTan97 I actually can't figure out what's going on here. I see the field in Django Admin, but I can't seem to get it from views.py
